-Full name (as appears in passport in block letter ) 全名(跟护照上写的要一样)
Gender Male Female (性别 : 男 or 女)
-Nationality (国籍)
-Date of birth month year (出生年月日)
-Place of birth (出生地)
-Passport Number (护照号码)
-Date of issue month year (护照签发日期)
-Occupation (职业)
-Identification of means of transport (交通工具写"飞机航班")
-Address of stay in Viet Nam (停留在越南的住址)
-Name, date of birth of children accompanying bearer (同行小孩的姓名跟生日)

-Purpose of Entry –Exit (出入境的目的一般写 1或 6)
1.Business/ Investment (经商/投资) 2.Employment (受雇)
3.Conference (会议) 4. Family visit (探亲)
5.Journalism (新闻采访) 6. Tourism (旅行观光)
7.Study (读书) 8. Others (其它)

-Any of the following symptoms:fever,cough, dyspnea, diarrhea,nauseous feeling,jaundice? Yes No
(有没有下列症状 : 发烧.咳嗽.呼吸困难.腹泻.呕吐.黄疸..)
-Date month year (当天日期)
-Signature of declarant : 签名(最好写英文)

FOR CUSTOMS DECLARATION (申报关税填的)

-Any unaccompanied baggages? (有无未随身(托运)行李)
Bringing over US$7000 (or other foreign currencies of equivalent value)? (有无带超过7000美金或等值外币)
-Total amount (总额)
-Bringing over VND 15.000.000 (超过 1500万越盾?)
-Total amount (总额)
-Bringing over 300g of gold ?(超过 300公克黄金?)
-Total weight :
-Temporarily imported anh reexported goods or vice versa ? (临时性带入带出的申报商品或违禁品可以填例如 : NB .相机之类)

-Dutiable goods: (应税货物)
-Date month year (当天日期)
-Signature of declarant : 签名(最好写英文)
